CHN receives referrals/questions from patients and their families, but also hospitals, nursing homes, assisted living facilities, and community organizations.

CHN is owned by Family Health Services of Darke County. Our relationship with our parent company affords us the opportunity to access all of their services to help our patients remain safely in their home – including home visits by a physician or nurse practitioner for those patients that have difficulty getting to their physician’s office.
